FDA milestones: the revenue and R&D switch

Here the regulator moves actual money.

Revenue recognition timing

You cannot recognize product revenue for a drug the FDA has not approved. Approval flips the switch. A sector quirk worth knowing: under US GAAP, R&D is *expensed as incurred*. A biotech spending $300M developing a drug shows $300M of expense and essentially no matching asset, even if the drug is worth billions. The balance sheet looks empty. This is why biotech valuation lives in pipelines and cash runway, not book value.

Financially, MDR/IVDR shows up in three places:

1. Rising compliance operating expense. Companies must re-certify existing products, expand clinical evidence, and pay Notified Bodies (private organizations authorized to assess conformity). Note that Europe's drug approvals run through the EMA (European Medicines Agency), the drug counterpart to MDR/IVDR's device world. EMA approval flips the same revenue switch as the FDA, just for European sales.
