In 2012, patients in the United States began developing fungal meningitis after receiving steroid injections for back pain. Over 750 people fell ill and more than 60 died. The cause: contaminated injectable steroids made by the New England Compounding Center (NECC), a Massachusetts pharmacy operating outside the standard oversight applied to full-scale drug manufacturers. Mold had gotten into "sterile" vials because basic contamination controls, the kind spelled out in manufacturing quality rules, were not being followed.

This is what GxP failure looks like in practice. Not an abstract compliance checkbox, but a preventable chain of events that ends in a hospital bed.

What GxP actually means

What compliance looks like day to day

For a pharma company, GxP is not a once-a-year audit. It is embedded operational discipline:

  • Deviation management: any departure from a written procedure (a temperature excursion, a missed signature) must be logged, investigated, and root-caused. Unresolved deviations are one of the most common findings in FDA warning letters.
  • Batch records: every step of manufacturing a drug batch is documented in real time. If a regulator asks "prove this specific batch was made correctly," the paperwork must answer instantly.
